Death & Birth

Death and Birth certificates maybe obtained by the kins of deceased patients or parents of the children born from the hospital on all working days at Room No. 25 of the Old Building.

Terms and conditions:–

  • Birth certificates for the children born only after 15th May, 2015 at DDU are provided by the hospital. For the children born before this date, birth certificate maybe obtained from MC Shimla.
  • Copy of Discharge card and parents’ Aadhar Card shall be produced for Birth certificate of Child.
  • For Death Certificate, Copy of Aadhar card of the deceased is mandatory.
